Please provide the following information:
- - your full name.
- - a telephone number they can contact you at (and/or e-mail address).
- - your certificate number.
- - name and telephone number of the hospital.
- - name and telephone number of the referring doctor.
- - diagnosis of your condition.
- - approximate number of days of hospitalisation needed.
If no appropriate medical facility is available at your current location, please contact the Helpline. In such an emergency the Helpline can utilise ‘International SOS’ to arrange an evacuation to an appropriate medical facility for your illness or injury.
The Helpline is available 24 hours a day, 7 days a week. For any other general advice or assistance (non-emergency) please contact us.
